  The address needs to be broken down into street, town or similar. Name should be broken down into Surname and Forenames. Some of the sections are confusing and it is not obvious what is required. The order of the items is not always logical. The most important information should come first.

S and E are the initials of the first name.

Compare the answer with forms collected from real organizations. Items should be in a sensible order with important items near the top of the form.

  Any two from: There must be a fixed length: They must look like what they are supposed to represent; They should be as short as possible in order to reduce the time to input them. They should have a consistent format.
