There should be a place for the patient's name and possibly their patient number (in case there are two patients with the same name on the same day).

Once the number is changed from a 4 to a 9, all those cells containing formulae that refer to this cell will be automatically re-calculated using the new value. Hence the spreadsheet would show the new values instantaneously.
